Banyan is a national provider of disability management, physical assessment and employer services. As such, the collection and use of your personal information is fundamental to our operation. At Banyan Work Health Solutions Inc., each of our consultants and employees are governed by this privacy statement as it pertains to the manner in which they collect, use, retain and disclose your personal information. We are committed to fostering trust and confidence amongst our customers, clients, employees and consultants. The practice of Banyan Work Health Solutions in relation to handling personal information is as follows: Personal information means any information about an identifiable individual that is recorded in any form, other than information that may be found on your business card. Personal information collected by Banyan Work Health Solutions Inc. directly from our clients with their consent and other relevant stakeholders in service provision to our clients (with client consent). The purpose for which personal information is collected will be identified to the client and documented at the time of collection. Personal information will not be used and/or disclosed for any purpose other than that for which it was originally collected unless the individual is notified and his or her consent is obtained or the use is otherwise permitted or required as a matter of law. Only that personal information which is necessary for client service purposes will be collected, used and/or disclosed. A client may withdraw their consent to the use of their personal information at any time by contacting their Banyan representative at 1-866-226-9262. If consent is withdrawn, this may limit the services or products we are able to provide. In some circumstances, as required by law, withdrawal of consent may not prevent the use or disclosure of personal information by Banyan. Banyan Work Health Solutions Inc. has taken all appropriate measures and put in place controls aimed at safeguarding personal information. Any personal information is kept in a secure location in the field office or head office. If the information is in an electronic format, Banyan uses passwords and file encryption for any on-line activities. Our employees and service providers have access to personal information only for the purposes set out in this statement to enable them to provide our services. Authorized personnel may access this information, in connection with the purpose for which it was collected, on a strictly "need to know" basis. Each client is entitled to access his or her individual personal information in the possession of Banyan Work Health Solutions and to question the accuracy and completeness of that information unless disclosure of particular information would necessarily infringe the privacy rights of others (i.e., such as a provider of confidential references).
