Making a Referral

What can I expect once I have made a referral?

When our administration centre receives a referral, a consultant is assigned to the client, based on skills and geography, within 48 hours. Should there be any unusual circumstances (e.g. extensive travel involved to meet the request) we will ensure that you are informed at the outset. We forward the file to our consultant via express post, requiring two to three days delivery and our consultant makes telephone contact with the client within 48 hours of the receipt of the file.

Our consultant schedules an in-person meeting with the client within seven working days and prepares a preliminary report as a result of that meeting. We forward a final report to you, the customer, within 10 working days after our consultant’s first meeting with the client. We then await your instructions. Our consultant will not take action on recommendations outlined in the report without authorization from the payer. At Banyan, we recognize the need for the customer to maintain control over the direction of the case.

How do I proceed?

  1. Please download and submit a completed Referral Form OR send a letter of referral detailing your request for service and identifying the client’s coverage details. You may also dial 416-777-1520 OR 1-866-226-9262. A customized referral form can be forwarded to you by email from the administrative centre. Should you require help to complete the form, we will gladly provide assistance.

  3. Please note: We ask that you, the customer, notify the client that our services have been requested, prior to our contacting the client. This ensures an informed and more receptive client when we call.
